If you are employed, please provide the following supporting Documents:

  1. Completed Application for all Applicants
  2. Two (2) Most Recent Paycheck Stubs
  3. Copy of Driver's License
  4. Proof of Residency
  5. Proof of insurance

If you are Self-Employed, the following supporting documents are required:

  1. Completed Application for all applicants
  2. Six (6) Months of Bank Statements
  3. Copy of Driver's License
  4. Proof of Residency

Social Security Applicants are asked to provide the following:

  1. Completed Application for All applicants
  2. Copy of Award Letter OR Three (3) Months' Bank Statements
  3. Copy of Driver's License
  4. Proof of Residency
